SHARING RESOURCES WITH A COUPLE HUNDRED COLLEAGUES
“In the changing face of health care reform, we bring together over 200 colleagues to work in partnership and collaboration to help adults find the best possible person-centered care, all through their lives,” says Kate Granigan, Co-Chair of the Geriatric Care Managers New England Chapter (GCMNE) 2014 Conference. “By exchanging ideas and improving our access to information and resources, we can help each other meet the needs of adults and enhance their lives.”
Ms. Granigan is speaking about “The Many Faces of Care Management: Diversity, Unity and Collaboration in the Age of Health Care Reform”, the focus of the GCMNE Conference. Care Managers from across Massachusetts and New England come together to “understand the many faces and responsibilities of being a professional geriatric care manager.” One important faucet of their role is to guide adults and their family members to make choices for appropriate care that fits a person’s values, beliefs, and preferences. We wondered- what if a couple hundred of our colleagues were provided with effective health care planning information and Massachusetts planning tools which would enable them to assist hundreds more adults and families with health care planning?
That is what’s happening next week. In addition to co-chairing the Conference, Ms.Granigan is Vice President of Overlook CARE, a dedicated Honoring Choices Community Partner. Ms Granigan and GCMNE, an Honoring Choices Collaborating Member, extended its hand to Honoring Choices and to another Honoring Choices Community Partner, Dr. Elizabeth Collins, Medical Director of Palliative Care, Lahey Health, to deliver a key presentation at the Conference. In coming together, we hope that a couple hundred of our colleagues will gain valuable insights and practical health care planning tools to inform and help hundreds of adults and families in communities across Massachusetts.
