Circle Home (formerly the Visiting Nurse Association of Greater Lowell) provides home health care and hospice services for people of all ages and neighborhoods in Greater Lowell, Greater Lawrence, and southern New Hampshire. They offer care at home for people who are expected to fully recover from an illness or injury, for people who need treatment and teaching to manage a lifelong chronic disease, and for people with advanced illness who are seeking a peaceful life closure. Elder Services of the Merrimack Valley
Elder Services of the Merrimack Valley, Inc. is a private non-profit agency founded by local advocates in 1974 to provide resources, information and services for elders and adults with disabilities who reside in 23 cities and towns within the Merrimack Valley. The agency’s goal is to enable individuals to maintain their independence and promote good health and wellbeing. We have a variety of services and programs to support people and their wish to remain living safely and securely in the community. We are respected as a leader in the field of elder care and demonstrate our commitment to older adults through our advocacy, education efforts and innovative services. Each day over 8,000 elders receive services or support from programs either managed or funded by Elder Services of the Merrimack Valley (ESMV). Most of our programs and services are targeted to elders in the greatest economic and social need. We have trained over 110 ESMV staff as Health Care Planning Ambassadors.

Greater Lynn Senior Services (GLSS) is the nonprofit Aging Services Access Point and Area Agency on Aging has been serving people age 60 and older and adults living with disabilities in Lynn, Lynnfield, Nahant, Saugus, and Swampscott, MA. GLSS serves over 4000 people every day with its consumer focused approach to promoting good health and well-being, providing meals, transportation, and home care services, and creating more livable communities for all. Among its many programs, GLSS offers the innovative Kiosks for Living Well, which are mobile, welcoming and colorful physical spaces that easily embed in places where folks in the community naturally go, like community centers, health centers, housing complexes, and so forth. Building on the concept of a “health center without walls”, the Kiosks incentivize consumers to become more engaged in health self-management by offering them easy access to a fun and comprehensive virtual technology (iN2L) as well as to nurses, social workers, falls prevention specialists, and peer advisors-right there at the site!—who deliver health education and monitoring supports in ways that “painlessly” help improve health behaviors and overall health outcomes. It’s a one-stop health education marketplace!

The Independent Living Center of the North Shore and Cape Ann, (ILCNSCA) is a service and advocacy center run by and for people with disabilities. ILCNSCA supports the struggle of people who have disabilities to live independently and participate fully in community life. “We welcome people of any age, with any type of disability, who live and work or wants to live and work in the 25 communities between Melrose and Rockport”, says Mary Margaret Moore, Executive Director. Lowell General includes the Honoring Choices Health Care Proxy and access to other planning information into their Admissions Packet. All patients who are admitted, in extended stay in the Emergency Department and in OB/Maternity will receive the health care planning information. Lowell General is also providing the planning information to all doctors and care processionals through their intranet. Dr. Tina Waugh, a family practice practitioner and primary care physician, helps to run a busy and diverse family practice group in Beverly with Dr. Gregory Snyder and Dr. Gail Ellis. Dr. Waugh makes health care planning a routine part of her practice because, as she says, “I see it as part of my checklist at the physical exam- check the vital signs, any changes in the family’s medical history, diet, exercise,”and I also ask, “have you done health care planning? ” Dr. Waugh has this conversation with every patient, 18 years old and older, at their annual physical examination. Choosing to make this conversation a routine part of the annual exam is rooted in Dr. Waugh’s strongly held belief “that every patient should have a Health Care Proxy, so I know who you want me to talk to if I can’t speak to you myself.” She also believes that by talking about this during every physical exam, the topic becomes something that patients expect her to talk about, and feel comfortable bringing up at other visits if they feel the need to. Dr. Waugh welcomes patient questions about making a health care plan and their preferences and choices for care. She talks with each patient about where to find planning documents like a Health Care Proxy and personal directive, and how to fill them out. Patients back bring the completed documents and Dr. Waugh scans a copy into their medical records. She checks with the patient to see if there have been any changes in their medical condition or preferences for future care, and if so “we can make changes and keep the documents very current.”
