A TOOL-KIT FOR CARE PROFESSIONALS

Our first Care Professional Community Partner, Kate Granigan, MSW, LICSW, is an experienced advocate in the elder care community, as well as an author whose article Care Providers: Important Members of Your Health Care Team frames the various roles of care providers, explores the various types of care they can provide, and maps the continuum of services necessary for a person’s changing health care needs.

In choosing to be a Community Partner, Kate recognizes “Honoring Choices as a great tool to have in my toolbox to help direct and assist my clients in finding the right care.” She also notes that the Honoring Choices model “is a unique resource that offers information and planning tools that are helpful to my clients in defining and honoring their care goals.” In advocating for individuals and families, Kate believes it will help “empower my clients to make meaningful choices for the best quality of life at every stage.”
