We’ll introduce the new “Health Care Planning Road Map” which outlines the Honoring Choices MA structured approach to health care planning, and includes the types of conversations, tools and discussion guides care providers can initiate at each phase health.
Rachelle Bernacki, MD, MS, the Associate Director of the Serious Illness Care Program at Ariadne Labs, and Director of Quality Initiatives, Psychosocial Oncology and Palliative Care at the Dana-Farber Cancer Institute, and Francine Maloney, MPH, an Implementation Specialist on the Implementation Platform, Ariadne Labs, will focus on facilitating appropriate conversations between clinicians and seriously ill patients and their families. Dr. Bernacki will provide a comprehensive review of Ariadne Labs “Serious Illness Conversation Guide”. Participants will learn how to start difficult conversations to better align care to a person’s values, goals and choices. In this webinar, we will:
• Offer interdisciplinary providers a structured approach to planning discussions, that begins at 18 years old and
continues through adulthood;
• Provide a road map of the types of discussions on the journey that outlines who, when, and how to have these discussions;
• Describe the “Serious Illness Conversation Guide”, a structured, evidence-based approach, for patient-clinician
conversations about values, goals and choices for patients with a serious illness.
