Beth Israel Lahey Health Primary Care Partners with Honoring Choices MA

We are excited to welcome Beth Israel Lahey Health Primary Care (BILHPC) as an Honoring Choices Community Partner.  BILHPC is the first major primary care organization partnering with Honoring Choices to routinely engage their patients in early and high quality care planning conversations.

BILHPC’s Senior Leadership Team and Primary Care Leadership Council have sponsored the movement of advanced care planning (ACP) to be normalized as a value in the BILHPC model for care. The BILHPC ACP Task Force has been activated to advocate for and organize initiatives that increase ACP awareness in primary care to benefit our patients and their loved ones.

Honoring Choices was delighted to start off our partnership by offering the popular Health Care Planning Ambassador Webinar to BILHPC Medical Assistants and other care team staff to confidently open simple planning conversations to complete a Health Care Proxy. Medical Assistants are an invaluable part of the care team and often the first person the  patient encounters in a primary care visit. We look forward to supporting everyone on the primary care team with proven tools and informational webinars.
