Honoring Choices tailors a program for each Partner to educate your staff and incorporate our educational tools into your existing programs to support and enhance your mission. Being a Partner includes:

• The Who’s Your Agent?^® Program, and Partner-only tools, staff training for care-provider to consumer discussions;
• Offer an Ambassador program for peer-to-peer discussions;
• Post your organizational/professional name on our "Meet the Community Partners" web page;
• Post short news articles in our "News/Events" web page;
• Post your "Partner Happenings" in our monthly newsletter;
• Region-wide networking meetings in the Fall and Spring;
• "Health Care Planning Facilitator Course"( fee based/optional)

It is a simple step-by-step planning process that helps adults explore up-to-date information to make care choices, view the 5 MA planning documents to write down choices, and learn how to put your plan into action to get quality care that honors your choices today and everyday!

You can help consumers to start to make a written health care plan with the Who's Your Agent?® Program. It takes all the information in the 3-Step Planning Guide and puts it into 2 easy-to-use tools kits, the Getting Started Tool Kit and the Next Steps Tool Kit. Consumers and care providers can have high-quality effective discussions at every phase of health.  Our "5  Things to Talk About" is a series of discussion guides that fosters routine health care planning discussions that both a consumer and care provider can initiate.

• For Consumers: 5 Things to Talk about With Your Care Providers is based on a consumer's 5 top rights to direct your health care. Consumers can be an adult, family member, Health Care Agent, Guardian and Caregiver.
• For Care Providers:  5 Things to Talk About With Your Patients & Clients mirrors the questions in the consumer guides from a care provider perspective. Care Providers are physicians, nurses, social works, all care professionals.

We partner with community groups, health care providers, care professionals and faith based & cultural groups — any care providers who can engage adults in health care planning and get them connected to care in their community. Partners offer a wide range of services to help adults & families at different phases of health. Some Partners can help to start a planning discussion; other Partners can provide care and services. Adults and family members contact a Partner directly to determine if their help & services are complimentary to what they need.
