Join us for a unique learning opportunity as experts in the California Medicaid Managed Care Palliative Care mandate present key implementation issues and lessons learned in delivering community-based palliative care services to a diverse, inclusive Medicaid population.
Our guest speakers Katherine Kerr and Loren Pogir of Transforming Care Partners and Dr. Anne Kinderman, Senior Advisor, Palliative Care Clinical Lead, will lend their deep expertise in the design, implementation, and evaluation elements essential for a successful community-based palliative care Medicaid benefit.
Presentation includes:
- Key areas critical to the delivery of palliative care to a Medicaid population, inclusive of historically marginalized populations;
- Lessons learned from California’s implementation;
- Features of CBPC benefits designed by other states and groups;
- Dismantling barriers and promoting the uptake of a Medicaid palliative care benefit;
- Building collaborative partnerships across diverse home and community settings.
