HONORING CHOICES HOSTS NATIONAL HEALTHCARE DECISIONS DAY

Honoring Choices Massachusetts is pleased to be selected as the State Liaison for April 16, 2015 National Health Care Decisions Day. To recognize the day, we invite community groups, care professionals, and health care providers from across the Commonwealth to join us for a community networking conference,

“Working Together: Building a Strong Network of Person-Centered Care in Your Community”

Come share your experience, receive educational materials, and build collaborative networks that promote wellness and person-centered care in your community. Participants will have an opportunity to network with representations from a diverse cross-section of the community: clergy and lay leaders, educators, nurses, social workers, physicians, attorneys, care providers who helps seniors, care professionals helping individuals diagnosed with cognitive and physical impairments, hospital personnel, hospice care professionals, and many more. The Conference will offer participants the following:

  •  Information and Massachusetts tools. Help engage adults in the health care planning process, and  open the door to lifelong care planning discussions;
  • Share Strategies that Make a Difference. Hear from our Community Partners running successful programs in diverse settings;
  • Build Your Community Network. Work together to broaden your network of coordinated, person–centered care for adults and families in your community.

In addition to the conference, Honoring Choices will post health care planning information and tools for community organizations to host their own activities in celebration of the day.  More information and registration for the conference will follow shortly.
