MAKING AN IMPACT

A year ago we began with a mission. Today, with the combined efforts of  30 extraordinary Community Partners, committed to extending health care planning to every citizen of the Commonwealth, we’re very please to add our “Impact” page to this website.

The “impact” that our Partners have had across the Commonwealth has been significant, as demonstrated by the graphic. Working with community, faith based and senior groups, care professionals and health care providers, we’ve been able to build customized health care planning programs for each Partner; all working toward the same goal but each in their own unique way. Each Partner is building a community network, reaching out to share resources & to coordinate care for adults in its area.

We are grateful to these Community Partners, each of whom who took a leap of faith with us and helped pioneer this highly collaborative initiative. Thanks, as well,  to our Collaborating Members, and  to the state and national organizations who share in this mission and provide additional tools and services.  We’re also indebted to our Contributing Professionals, experts and local leaders who generously offer information to help adults & families explore their legal rights, medical information, and values and beliefs on their personal journey.
