Welcome Point32Health to the Honoring Choices Partner Network!

We are delighted to partner with  Point32Health  and support their mission.  Point32Health— the parent organization of Harvard Pilgrim Health Care and Tufts Health Plan — is working to guide and empower healthier lives for everyone. As a result, Point32Health will launch a crucial Advance Care Planning intervention that ensures members’ health care goals are understood and documented to make sure the care they receive is the care they want. Honoring Choices MA (HCM) will support Point32Health’s Advance Care Planning intervention for its members, care teams, and everyone in our diverse communities. Advance care planning is an essential tool in empowering members throughout their health care journey. Through the lens of health equity and addressing social determinants of health, Point32Health’s interdisciplinary care team model aims to:

• Support advance care planning across all populations
  to enhance the quality of life
• Ensure members have advance directives in place
  to promote care plan alignment with member wishes
• Identify members who could benefit
  from an advance care planning conversation
• Understand when and how to facilitate a referral
  to palliative care service or hospice care

Honoring Choices is working closely with the Point32Health teams on a comprehensive plan to provide shared access to the signature Honoring Choices MA Who’s Your Agent? Program, multilingual consumer friendly tools kits to make a plan, and staff training to engage adults in planning conversations- from simple to serious illness- for  good care today and over your lifetime.
